Is Hospice the Right Choice?Nursing Care
We have an experienced team of Nurses who are experts in symptom and pain control and serve as the case manager for each patient. They collaborate with the entire hospice team on a specialized plan of care that covers medications, equipment, personal care, as well as the spiritual and emotional needs of the patient and their caregivers. Nurses typically visit 1 to 3 days a week, however, our patients and families have access to our on-call nurses 24 hours, 7 days a week.
Personal Care
Our specially trained hospice aides work under the supervision of our RN case managers. They provide personal care to our patients based on their needs. This may include such things as bathing, dressing, grooming, skincare, bed linen changes, light housekeeping, and socialization. Aides typically visit one to three times a week depending upon the patient’s needs and plan of care.
Counseling & Social Services
At TruHeart Hospice, caring and knowledgeable social workers are deeply dedicated to providing the emotional support, counseling, and community resource information needed by our patients and their caregivers. They assist with advanced care planning, financial assessment, and housing or facility placement. Their fundamental goal is to be a resource for the family and to empower our patients to find comfort, peace, and a sense of fulfillment during their journey.
Equipment & Supplies
We provide all the equipment and supplies necessary to provide a safe, and comfortable environment in our patient’s home. Equipment might include a hospital bed, side table, bedside commode, wheelchair, and oxygen. Examples of supplies could be adult diapers, wipes, gloves, and personal care items.
Pain & Symptom Control
At TruHeart hospice we believe that helping you manage your pain and symptoms is more than medications. Our primary goal is comfort for our patients to ensure that they have the highest quality of life for as long as they live. Our team is specially trained in medical and alternative techniques and will work closely with you and your family to develop a specialized care plan to meet your needs.
Spiritual Care
Our Non-denominational Chaplains provide a ministry of presence and meet our patients and families wherever they are on their journey. They work hard to ensure that patients feel free to discuss their spiritual concerns, ask essential questions, realize the importance of their lives, and feel peace. Our chaplains also help facilitate visits by the patient’s local clergy if preferred.
