YourHospice Team
Each patient has a specialized team available to help implement and support their unique and specialized care plan:
- Physician: Patients have the right to choose their primary physician or use our hospice physician to oversee their care. The physician closely monitors the progression of the patient’s illness and works with the hospice medical director on development of a plan of care.
- Hospice Medical Director: Our expert hospice physician/medical director leads the hospice team in developing a plan of care specific to the patient's needs, provides consultation to physicians concerning hospice care, and certifies terminal prognosis for hospice eligibility.
- Nurses: Each patient will have an RN and LPN team providing holistic care to the patient and family. This includes completing assessments, managing medications, and providing education about interventions and what to expect. They also serve as the manager of the patient’s plan of care and point of contact for the patient’s family and physician.
- Social Worker: Our Social Workers provide emotional support, counseling, and community resource information. They can help with financial assessment and planning, advanced care planning, housing and/or facility placement, and work to ensure that all the patient’s needs are met in a timely and professional manner.
- Hospice Aide: Our hospice aides are specially trained to provide personal care based upon the patient’s needs and plan of care. They aid with bathing, grooming, shaving, skincare, and dressing, as well as bed linen changes, light housekeeping, and socialization.
- Chaplain: Each patient is offered spiritual care from a non-denominational chaplain who can provide spiritual and emotional support ministering to the whole person, their family, and caregivers. They go alongside the family, meeting them where they are on their journey. They can also help facilitate visits by the patient's or family’s preferred clergy.
- Volunteers: Our trained volunteers provide socialization and support to our patients and caregivers. experienced professionals ensure that medication regimens are followed accurately, promoting healing and well-being.
Volunteers
Volunteers play a vital role in hospice, from patient care to helping in the office. TruHeart Hospice provides all the training to help our volunteers feel prepared and confident.
Our volunteers are a crucial component of the support our hospice team is able to provide to hurting families.
Volunteer Opportunities
- Administrative: support in the office such as filing, mailings, answering phones, and clerical work.
- Direct patient care: visits to families and patients, caregiver relief, light housekeeping, walking dogs, etc.
- Community outreach: help organize/implement special events, holiday parties, carnivals, and community education.
- Bereavement: phone calls to families, help organize memorial services, prepare mailings.
- Tuck-in program: call patients/caregivers at the end of the week to check on supplies or needs before the weekend.
- Jr Volunteer Program: young people/youth groups to sing, paint nails, provide activities or crafts.
